Key Responsibilities:
- Provide ongoing clinical and psychosocial assessments of new patients, providing reassurance and building rapport
- Establish individualized care plans to meet specific health needs based on evidence-based standards
- Develop wellness goals by determining motivating factors and leveraging them for best possible health outcomes
- Identify and address barriers to care that have been identified by patients and/or the care team
- Reduce care gaps by frequent engagements with patients
- Assess the need for and educate patients on equipment to monitor their health remotely
- Conduct ongoing surveillance and timely response to abnormal diagnostic results
- Assess urgent concerns and proactively triage patients to support appropriate utilization of emergency services
- Document all care interactions and escalate to multidisciplinary teams as needed
- Support the development of programs and products by providing user feedback
Requirements:
- Registered Nurse with a minimum BSN, residing in a compact state, and possessing a current, active, unencumbered compact RN license with multi-state privileges
- 4+ years recent experience in a hospital and/or healthcare practice, serving maternal-child health populations (minimum 3 years in labor and delivery)
- Flexible and agile thinker who embraces change
- Internet-connected, able to work remotely via video, phone, and text
- Willing to travel occasionally (infrequent)
- Willing to work weekends and holidays
- Understand the prevalence and role of birth inequity and structural racism in maternal morbidity and mortality
- Professionally engaged
Preferred Qualifications:
- Experience working with perinatal patients who have had pregnancy-related diseases such as diabetes, hypertension, perinatal loss, etc.
- Experience providing virtual care
- Spanish fluency (without the use of an interpreter)

Compensation:
The expected base salary range offered for this role is $78,000-$83,000. This role is also eligible for equity, giving you an ownership stake in Pomelo's mission. Actual compensation may vary based on relevant experience, skills, competencies, and certifications.
