- 7:00 AM – 7:00 PM OR

Perfect for nurses seeking consistent weekend shifts while providing meaningful, one-on-one patient care.
🧡 What You’ll Do
- Provide skilled nursing care for a patient with a tracheostomy, ventilator, and PEG tube.
- Monitor and assess the patient’s physical, emotional, and clinical condition.
- Execute the patient care plan and communicate effectively with the RN Case Manager.
- Administer medications and treatments as prescribed and authorized.
- Assist with tracheostomy care, ventilator monitoring, and enteral feeding support.
- Provide personal care, including bathing, grooming, dressing, toileting, and hygiene assistance.
- Assist with mobility, positioning, transfers, and use of medical equipment, including a Hoyer Lift.
- Prepare meals and assist with eating and hydration as appropriate.
- Follow nutritional guidelines and care instructions established for the patient.
- Perform light housekeeping duties within the patient’s living area.
- Help maintain a clean, safe, and comfortable home environment.
- Accompany the patient to approved medical appointments and community outings as authorized.
- Observe and report changes in condition promptly to the care team.
- Complete accurate and timely documentation using the Agency-approved Electronic Visit Verification system.
- Demonstrate accountability to assigned schedules and communicate scheduling needs to the Staffing Coordinator.

Requirements
- Graduate of an accredited Practical Nursing (LPN) program.
- Current Pennsylvania LPN license in good standing.
- Ability to read, write, and communicate effectively in English.
- Ability to understand and follow written and verbal instructions.
- Successful completion of a job-specific knowledge examination.
- Strong organizational and time-management skills.
- Compassionate and professional attitude toward the care of the sick and medically fragile.
- Ability to document care with detail and accuracy.
- One year of home health experience preferred.
- Experience with tracheostomy care, PEG tubes, and ventilators strongly preferred.
- Familiarity with Hoyer Lift transfers and other assistive devices.
- Strong observation, assessment, and reporting skills.
- Ability to work independently in the home setting.
- Employees designated in driver status must maintain a valid driver’s license and auto insurance at all times.
