Liberty Community Program is a leading organization dedicated to providing compassionate care and support for individuals with autism and developmental disabilities. We are currently seeking a skilled Nurse Practitioner to join our dynamic team. The Nurse Practitioner (NP) will play a pivotal role in assessing, diagnosing, and managing the healthcare needs of our clients. This position is essential for bridging the gap between patient needs and clinical services in a supportive and collaborative environment.
The Nurse Practitioner will conduct comprehensive health assessments, develop treatment plans, and provide ongoing monitoring of chronic and acute conditions. The ideal candidate should possess a deep understanding of behavioral health and be passionate about improving the quality of life for those we serve.
Key Responsibilities:
– Perform thorough physical examinations and health assessments for participants.
– Develop and implement individualized care plans based on assessment findings and client needs.
– Diagnose and manage acute and chronic health conditions.
– Prescribe medications and monitor their effects.
– Provide education and support to clients and families regarding health management and wellness strategies.
– Collaborate with a multidisciplinary team including physicians, therapists, and direct support staff to ensure coordinated care.
– Maintain accurate and timely documentation of patient interactions and care plans.
– Conduct follow-up visits to assess progress and adjust care plans as necessary.
Requirements
– Master’s degree in Nursing or Doctor of Nursing Practice (DNP)
– Valid Nurse Practitioner (NP) certification and licensure in the state of West Virginia.
– Experience in a healthcare setting, with a focus on behavioral health or developmental disabilities preferred.
– Strong clinical assessment skills and ability to make independent medical decisions.
– Excellent communication and interpersonal skills.
– Familiarity with electronic health record (EHR) systems.
– Ability to work collaboratively with a multidisciplinary team.
– Valid CPR/ BLS certification.
– Commitment to providing high-quality care and advocacy for individuals with disabilities.
– Must have reliable transportation and the ability to travel for services as needed.
– Preferably familiar with drug addiction and child services.

Salary: $114,060.00 – $120,163.00/ Year
